Palm Coast man, 25, dies in three-car crash in St. Johns County

Palm Coast resident Jose E. Vazquez-Trujillo, 25, died after a vehicle he was riding in crashed on Interstate 95 in St. Johns county early the morning of Jan. 10.

Vazquez-Trujillo was the sole passenger in a 2004 Toyota Corolla driven by 25-year-old Sanford resident Dhayre M. Olivo Oquendo, according to a Florida Highway Patrol report.

Olivo Oquendo was heading north in the right lane of I-95 at about 4:48 a.m. Jan. 10 when she switched to the center lane and then the left lane, then lost control of the car and traveled onto the median, striking a guardrail, according to the report. 

The car spun back into the roadway into the path of a 2017 Chrysler Pacifica driven by Quebec resident Jonathan Dionne, 39. The Pacifica's left front side struck the passenger side of the Corolla, which spun again and was hit by a 2013 Nissan Altima driven by Ormond Beach resident Robert Brown, 53, before hitting a concrete barrier.

Vazquez-Trujillo died at the scene of the crash, and Olivo Oquendo was hospitalized at Halifax Health Medical Center with serious injuries. Neither of them were wearing a seatbelt, according to the report.

Dionne and an adult female passenger in the Pacifica were not injured, but two girls in the vehicle, ages 6 and 8, had minor injuries and were hospitalized at AdventHealth Palm Coast. Brown was alone in the Altima and was not injured, according to the report.
